I don't think the edit of the boxart had anything to do with it being relation to gore, I read once before the Japanese have a hate-on for four-fingered hands or something (something to do with slaves, they used to cut off a finger or something I don't remember), like with the Japanese version of Abe's Odyssey I think they had to add/take a finger from the Mudokons because they related it to the same shit.
