I think it's like:
His watch has stopped. He is not paying attention to the real pulse at all. The pulse only exists when it is measured by a watch in his mind. As his watch has stopped, it has stopped ticking, which transitively he associates with the heart, which also ticks, and assumed he is dead, instead of do what it seemed he was doing, a tachycardia test.
