Moonbase alpha glitch (I have no idea)

I was playing moon base alpha waiting for fax to join. I Was just walking and I saw this lady. The lady wasn't that big. She disappeared after I blinked. I was walking again and saw these lines on the floor. I looked at it carefully, and skulls went flying. I Was just walking again, and the lady popped up, then disappeared. I have no idea what this is, but if it's happened to you, let me know.
