For me, it depends on the story.

You are supposed to suspend belief if it's set in a supernatural setting where people turn into green hulks and alien gods carry hammers.

If it's set in 'the real world' (i.e. Bond movies) I tend to be more critical.

There are entire essays written on similar subjects. Basically you just need to have rationalizations for everything in fiction even though there are never any easy rationalizations in real life, kind of an interesting topic. And it's not as simple as you make it.
