Most likely cause was it was an airborne virus/bacterium that could only enter through the blood at first, since that is one way that most all STDs can be transfered, and evolved into one that can also be transfered through genital body fluids.

You get infections because of bacteria, the bacteria have evolved to survive on the human body, they could have started of as any type of bacteria maybe even a harmless one. But at one point it evolved into something else.  The same goes for viral stds, they could have started of has a harmless virus for most, but when introduced to a new host a virus can change (evolve) into a new virus. When having sex you exhange fluids which could contain bacteria or viruses, and so you can obtain an std.

Also, keep in mind that until this century the only real threats of STDs were the bacterial ones, like Syphilis and gonorrhea. Things like syphilis were actually deadly back then. With the explosion of population and the "sexual revolution" the number of identified STIs (which is actually the proper term for them) grew incredibly.

Yes, like I said, just blood/blood transfer can do it. That's why some people in poor third world countries that can't afford scanning equipment can sometimes give STIs to people through transfusions, or how junkies can pass it from sharing needles.
